Capturing User information in form central

Hi

I just created a questionnaire in form central and added to a website. Was wondering how I capture the user information of those completing tthe questionnaire. My goal is to capture the user information in the adobe catalyst CRM at the back end of the site. Would I need to create an intial form that redirects to the questionnaire? Or can it be done in form central and capture in both the questionnaire summary report and the catalyst CRM for the site.
